A friend of mine has recently had a quarter sized area form on their neck. Fearing skin cancer (very common in his family) he went to the doctor and the doctor stated it looks like a birthmark just starting and suggested not even bothering to spend the 90 bucks cash it would cost him (he has no medical insurance). He has obviously made an appointment with another doctor but do birthmarks really appear at times so "late" in life?
Sorry. The $90 is for a biopsy.

Can a birthmark seriously only show up on someone when they are in their 20's?

Sounds unlikely it's a birthmark, but more likely the doctor is lazy. He's making the correct decision in furthering the investigation.
