Should I be worried about this double ear infection?

About 3 weeks ago I had a severe ear infection. As I have no insurance, I went to the local clinic and was prescribed 875mg of amoxicillin, 800mg of ibuprofen and ear drops. The next day I ended up going for an antibiotic shot. The ear drops, however, kept going into my throat instead of staying in the canal(?), so I went back two days later. Another physician looked at me and said that it was very bad in there and that he could see some some "tube damage" or something like that but because I could not afford to pay for anything anymore at this point after the other expensive visit/medication/shot, the head nurse informed me that I was fine and to finish taking what I had. She was sure in a hurry to kick me out once she realized I had no more money, ha. So here I am again 3 weeks later and after the same ear infection coming back, finding old antibiotics and Motrin to relieve the pain, the other ear has joined the party (3-4 days of it now). Double whammy! I've been having constant headaches, uncomfortable discharge coming out from ears, swollen and very painul "balls"/lymphs? under my ears (to the point where it feels like the sides of my face are difficult to move nor can I close my jaw all the way and it's annoying to swallow.) What should I do now? ):

go to an emergency room, by law they have to treat you. You will get a bill, but you can make arrangements with the hospital you choose to make payments on it.
