The stress test is done to assess perfusion of different parts of the heart wall such as anterior, posterior or inferior walls. Often a small area that includes the apex of heart and a small portion of inferior wall collectively called 'inferoapical' area supplied by a branch of LAD artery may be partially or completely blocked.

In 'mild inferoapical defect' there is a partial block that reduces blood supply(perfusion) to that area.
The result is positive for mild IHD.
